Food as Medicine: How MSK Is Helping Latinx Cancer Patients Eat Right - On Cancer - Memorial Sloan Kettering

A healthy diet is known to help prevent cancer. But a healthy diet is also crucial after a patient is diagnosed. Cancer treatments can rob patients of an appetite, but they still need to eat well to be strong enough for treatment.

That was the challenge for 51-year-old Lidia Rodriguez, diagnosed in March with stage 2 breast cancer. She had surgery and radiation at Lincoln Hospital near her home in the Bronx and now takes Tamoxifen, a medicine used to treat breast cancer and prevent its recurrence.

Besides some lingering pain in her breast, she’s feeling good today. And through a joint partnership between Memorial Sloan Kettering Cancer Center and Lincoln Hospital, she’s picked up healthy new eating habits that will further aid in her recovery.

Fulfilling a Need

Food insecurity can make eating healthy difficult. More than 56% of NYC cancer patients surveyed by MSK say that they lack a consistent access to food, according to Francesca Gany, the founding chief of the Immigrant Health and Cancer Disparities Service at MSK. She connects underserved patients to health care access.

Lidia is part of an ongoing clinical trial led by Dr. Gany that will determine how to get nutritious food to patients who are food insecure. For the study, Dr. Gany and her team created healthy eating guides for Spanish-speaking breast cancer patients. They’re sharing the materials with patients like Lidia at local hospitals in underserved communities, including Lincoln, Montefiore Medical Center, and St. Barnabas Hospital in the Bronx.

Many existing materials are not tailored for patients like Lidia, a native Spanish speaker. An important requirement for the new materials is that they account for cultural nuances.

“We had focus groups that represented the full range of Spanish-speaking countries and regions within those countries,” Dr. Gany says.

Foods, too, were tailored to audiences. Dr. Gany said it was important to include dishes that were important to the various cultures of Latin America.

“We really wanted to make sure that the foods we were showing resonated across all of the different groups,” Dr. Gany adds.

Breast cancer patients were selected because the disease is the most common type of cancer, though Dr. Gany recently got approval to open the study to women with gynecologic cancers.

What’s Cooking?

Creating and sharing the eating guides is one part of the trial. Putting the information into practice is another. Each week, study participants receive groceries and use their newfound knowledge to prepare healthy foods. They also meet with Angelica Alvarado, the program’s clinical coordinator. She is bilingual and meets with participants for one-on-one nutrition counseling.

“We talk about salt intake, whole grains, nutrition labels, and what to eat to manage cancer-related side effects like nausea and bloating,” Ms. Alvarado says. “We also teach them how to shop at the grocery store for items that last longer and don’t cost as much, like beans and canned salmon.”

The advice benefits participants’ health and wallets.

“Undergoing cancer treatment can sometimes affect ability to work, and that’s very normal. With the pandemic, work has become less viable for many of our patients,” Ms. Alvarado says. “We want to provide support.”

Lidia has appreciated that support because she’s unable to work and no longer receives unemployment benefits. Through the study, she is able to receive the healthy items that are now part of her post-treatment care.

She has had no trouble incorporating new habits. She’s swapped canned fruits and vegetables for fresh ones, uses avocado oil in her cooking, and flavors her food with garlic and pepper, not salt. With guidance from Ms. Alvarado, she loves to make juices, oatmeal, eggs, and stewed lentils. She also now avoids fried foods.

She’s lost 15 pounds, and while she says some of that may be attributed to cancer side effects, she thinks the program played a role.

“I was a bit overweight and had high cholesterol, but I’ve lost weight and lowered my cholesterol,” she says. “I am so thankful for the program. It’s helped me quite a bit and I will continue to eat this way after it ends.”

The patient response has been “overwhelmingly positive,” Dr. Gany says. “Many say it’s the first time that they had ever been given cancer treatment nutrition information.”

Looking Ahead

As part of the study, Dr. Gany and her team are also learning how to best get the groceries into patients’ hands. Is it by having them pick up food at MSK’s food pantry? Getting food delivered to their homes? Or receiving vouchers to take to the supermarket? Patients are enrolled in the trial for six months, so there is ample time for the team to determine what’s working.

Ms. Alvarado says it can be hard to change habits, but through the program she’s seen it happen.

“Some of these things are ingrained,” she says. “It makes me very happy when I see patients try new things and really enjoy them.”

While it may have been something your mom told you as she tried to give you a bag of cereal as you walked out the door to school, it’s become a trend to intentionally skip a morning meal for health promotion in some cases, such as with intermittent fasting.

But what’s the real story? And how do registered dietitians advise their clients — especially those who say they just aren’t hungry in the morning? Here’s the scoop.

The Claim About Eating Breakfast

The origin of the original claim — that breakfast is the most important meal of the day — is based on a marketing campaign by a cereal manufacturer in the 1940s, as The Atlantic reported in 2016. The goal? To sell you more Grape Nuts, not to improve public health.

Since then, however, there has been a lot of research on how eating breakfast affects health and weight, as well as learning and cognitive performance in adults and children, says Ginger Hultin, RDN, owner of ChampagneNutrition in Seattle and author of Anti-Inflammatory Diet Meal Prep and How to Eat to Beat Disease Cookbook.

So while the origin of this claim wasn’t rooted in scientific evidence, there is now research to support the recommendation.

The Scientific Research on Whether to Eat Breakfast

While you can find studies supporting both sides of the debate, it’s important to focus on meta-analyses, which look at many studies on the same subject to find a conclusion, says Hultin. “The evidence seems to be most conclusive that skipping breakfast creates worse health outcomes for many people,” says Hultin.

For example, in a meta-analysis of 45 observational studies published in the January­–February 2020 issue of Obesity Research & Clinical Practice, researchers concluded that skipping breakfast is associated with being overweight or having obesity. One reason? Skipping breakfast, or not eating for extended periods of time, may increase overeating at other times of the day, says Hultin. “This is extremely common and actually a pattern that’s tied to worse outcomes for weight management, not better,” she explains.

The choice to eat breakfast doesn’t only affect weight — metabolic health is also involved. A systematic review and meta-analysis published in November 2015 in Public Health Nutrition concluded that skipping breakfast correlated to a 21 percent increased risk of type 2 diabetes, and that eating breakfast could be protective against developing the disease.

And then there’s mental health. “There’s research that ties breakfast skipping to a higher odds of being depressed and stressed,” says Samantha Cassetty, RD, nutrition and wellness expert based in New York City and coauthor of Sugar Shock. She points to a systematic review and meta-analysis published in Nutritional Neuroscience in December 2020 that involved nearly 400,000 people. The data linked skipping breakfast a 55 percent higher likelihood of having psychological distress compared with people who ate breakfast.

That said, studies are conflicting. Take a meta-analysis published in January 2019 in BMJ, which looked at 13 randomized controlled trials. Participants who were instructed to eat breakfast consumed about 260 more calories compared with those who were in a breakfast-skipping group. (That said, as the authors point out, the quality of these studies were low — for one, they were conducted over an average of seven weeks — so more studies are needed.) Another meta-analysis, published in Obesity in June 2020, also looked at randomized controlled trials, finding that breakfast skipping was associated with about a pound of weight loss over two months compared with eating the meal, but skipping is also associated with an increase in “bad” LDL cholesterol. Longer-term studies are certainly warranted.

You could say that skipping breakfast is really the modern-day intermittent fasting. A popular eating pattern is called 16:8, in which people fast for 16 hours and eat for 8 hours per day. Often, they will start eating at noon. This type of eating may offer health benefits, including improving metabolic health for those who have obesity or diabetes, decreasing risk of cardiovascular disease and cancer, notes a review published in December 2019 in The New England Journal of Medicine. The authors hypothesize that this may be because fasting may switch the body into a ketogenic state, encouraging it to burn fat for fuel, and improving blood sugar levels, blood pressure, and decreasing visceral fat. But more studies are needed, and what’s clear is intermittent fasting isn’t for everyone. What’s more, health experts warn the eating plan may be unsustainable and may set some people up for overeating later in the day.

Nutritionally speaking, skipping breakfast may not be wise, Cassetty points out. “One study, published in Proceedings of the Nutrition Society in April 2021, noted that breakfast skippers consume fewer nutrients, like calcium, vitamin D, folate, and iron. Even when snacking more, breakfast skippers don’t make up the nutrients from the missed meal,” she says. As it is, many people don’t have balanced enough diets According to the U.S. Department of Agriculture (USDA), when it comes to eating a wide variety of healthful foods, Americans received an average score of 59 out of 100 on the “Healthy Eating Index,” which assesses diet quality. If you choose to skip your morning meal, you’ll really need to make sure that lunch, dinner, and snacks are on point.

It's also important to put this question into context. “There are many other factors that play a role in one’s risk of developing certain chronic conditions, aside from whether or not a person eats or does not eat breakfast, so it’s difficult to find conclusive evidence of one specific habit without looking at the bigger picture,” says Elizabeth Adrian, RD, founder of City to Sea Nutrition, an in-person and virtual nutrition counseling in New York City. Still, she recommends her clients eat breakfast to regulate hunger, prevent overeating, and get in a quality meal at the beginning of the day.

The Final Word on Whether You Need to Eat Breakfast

All three registered dietitians we interviewed recommend breakfast to their clients. But what you eat matters more than the specific timing. It’s okay to wait until you’re hungry, even if that comes later in the morning. Focus on whole, plant-based, fiber-filled foods and limit added sugars, refined grains, and excess sodium, says Cassetty. Scrambled eggs with veggies and whole-grain toast or a smoothie made with fruit, veggies (like spinach), nut butter, almond milk, and protein powder or Greek yogurt are two examples of power breakfasts that fit the bill.

Why your kids should eat school lunch. Even if you can pack it for them - WBUR

Attention all Massachusetts parents and caregivers: Thanks to a federal flexibility waiver through the U.S. Department of Agriculture (USDA), school meals are free for all students this year — and your child should eat them. This is our opportunity to demonstrate that school meals for all is a powerful solution to ending childhood hunger.

The federal waiver, which is part of COVID relief funding and expires at the end of this school year, is a dramatic departure from the pre-pandemic system, which relied on a tiered eligibility scale. Under that system, some kids paid the full cost of a school meal and some paid a reduced amount, while others ate for free. When students are forced to pay for school meals, we perpetuate stigma around family income status.

The previous system also didn't meet the need. In 2019, 26% of children experiencing food insecurity in Massachusetts couldn't get free or reduced-price school meals through the federal program. Our priority must be to feed our kids, full stop, and universal school meals will do that. When all students eat for free, school meals transform from being a stamp of poverty to a tool for community building and social development. That’s especially true now, 18 months into a pandemic when opportunities to do so are increasingly difficult. In 2020, 28.3 million meals were served to kids and teens at no cost through federal waivers, and Project Bread's FoodSource hotline calls increased to more than 46,000 calls.

Melissa Campbell, a graduate student in social work at Salem State University and an advocate for universal free meals, was homeless for a short time in high school. At a recent hearing, she urged Massachusetts legislators to make free meals a reality for all students.

At 15, buying lunch for $2.50 every day was so much easier than trying to prove I was poor enough to qualify for a free meal. The thought of having to look a bunch of adults in the face and explain to them why I should be able to eat for free was terrifying and humiliating. Instead, I did what made the most sense in my teenage brain: I ran up a huge lunch debt. Then, every few weeks when I got paid, I’d slip the lunch lady a $20 bill and hope no one would notice.

A mountain of research tells that hungry students can’t learn. We also know that schools — principals, teachers, school nutrition staff — want their students to have enough to eat. At Project Bread, we see how school districts are working to develop new recipes that use common USDA foods and pioneer strategies like recipe testing with kids, to ensure school lunches include textures and flavors students actually want to eat.

When all students eat for free, school meals transform from being a stamp of poverty to a tool for community building and social development.

Districts are reimbursed for school food based on participation. When more kids eat free breakfast and lunch, more federal funding comes into the district. For example, when Springfield Public Schools adopted School Meals for All, they saw a 43.5% increase in school food program revenue between 2014 and 2017, enabling them to make several improvements to quality, including sourcing whole ingredients and leveraging a new culinary and nutrition center. This additional revenue affords all schools greater flexibility to invest in student nutrition, whether that’s in the use of high-quality ingredients, being able to meet rising food prices, or having greater capacity to hire and train more school nutrition staff. The funds also support a school’s ability to offer culturally relevant meals, reduce food waste and expose students to new foods they may not get at home.

When participation in school meals is low, less revenue flows into the school districts, which perpetuates the cycle of underfunded school nutrition departments that must work twice as hard with less staff to continue to provide quality meals for all students. If districts can demonstrate high participation, we all benefit.

The media has highlighted supply chain issues and labor shortages that are impacting school meals, such as a major produce distributor closing in Watertown last month, resulting in at least 90 school districts having to find a new source right as the school year began. But families should not misunderstand this coverage as a reason to opt out. Quite the opposite. More children eating at school is part of the solution.

When a student visits the school nurse for a scraped knee, they aren’t asked to pay for the bandage. And school libraries don’t charge an entry fee at the door. The cost of educating future generations is understood to include first aid supplies and books. So, why is meeting one of our students’ most basic needs — food — any different?

In addition to advancing a proven solution to address childhood hunger, mitigating stigma and providing better financial stability for your school district, having your child eat free meals at school helps picky eaters and makes mornings for families just a little bit easier.
